Hi Chris,
Cygnus provides employees with a choice between Aetna's PPO and HMO plans.
If you are located outside of California, only the PPO is available to you
though (I don't know which office you will be working out of).  I am not
sure how familiar you are with HMO and PPO plans - I will Airborne you a
packet of info. and we can review it over the phone on Tuesday morning
together.  Basically, the HMO has no deductibles, most services are covered
100% after the copayment of $5 is made, and you have a primary care
physician which you go to for _everything_.  The PPO has a yearly $250
deductible, coverage is 100% for basic doctor visits after a $10 copayment
and 90% for lab work, x-rays, hospital visits, etc., and you can pick any
doctor in the network to see at any time (for example, you can see a
dermatologist one day, an internal medicinist the next, and a neurologist
the next).  Both plans are $1/month for a single employee.  

Regarding preexisting conditions:
1) If you have been covered by an insurance company for the preexisting
condition for at least 12 months prior, your preexisting condition will
continue to be covered.
2)  If you have not had prior coverage for 1 year, Aetna will not cover you
for the condition for 6 months.  After the 6 month waiting period, you will
be covered for the condition.

You are eligible for all benefits as of your start date.

Our dental plan is as follows:
-All dentists are covered
-Preventive covered 100%
-Basic covered 80%
-Major covered 50%
There is a $50 calendar year deductible for basic and major work only.  
The monthly cost for a single employee is $6.93.

We also have short term, long term disability, and life insurance.

Information on the 401k plan will be included in the pack of info. I am
going to send right now.
