The program was operated under several names, including Physicians Benefit Plan, Physicians Choice Plan, Golden State Preferred Trust and Unified Benefit Plan. Garamendi is also seeking revocation of the license of benefits administrator Robert Turell of Insurance Design Administrators, whom he accused of aiding the scheme.
