Sample rate conversion needs to be handled.

Some compressed data formats need to be handled.

Whenever a file is opened for writing, the file header should be
written immediately (rather than when data is written to the file).

A few other weird things aren't implemented.

These problems will hopefully be resolved in version 0.2.

PCM mapping will be handled when I have nothing better to do.
