Subject: questions for form

1. MIT Phone:
Whose phone should we use? It doesn't mention anything about being put
in the phone book, but it sounds like a good idea if it's not done
automatically.

2. MIT Address: I'm putting W20-401. We'll get a mailbox. It means we
should decide on a person to pick up mail from there regularly. Any
objections?

3. email:
We should put one person's email. It should probably be the same person
for the MIT Phone: in #1 above. The purpose for this is to get announcements
about ASA meetings and such. Hence, we should not put "sps" on here or
EVERYONE on sps will get the notice.

4. Number of members:
I don't know how many we have. Furthermore, are alumni members, too?

5. Advisor:
What should we say? Kim Wainright? Our previous advisor was Prof. Redwine
(x3-3766). Never heard of him. We might want to talk to him when we figure
out what we want to say.

6. Non-MIT affiliations: What's the national organization called?

7. Signatories: Who should be on this? (max. 5) These are people who
should be able to reserve rooms/drop poster space, etc.
