  Please try to read this news article in preparation for tomorrow's class:

It's about a recent medical device that overradiated many patients with tragic results. As you read the article:
- Mark the problems/issues that you think contributed to the disaster.
- If you find the root cause of the accidents, mark that as well.
- Note any similarities to Therac-25

You don't need to hand anything in, but part of tomorrow's class will involve going around the room to discuss what you found.

Write-up excerpts

Hi Everyone,
  I've been going through the design write-ups, and I've seen a lot of great comments / critiques of these papers! Unfortunately, you guys don't get to see any examples other than your own submissions. I'd love to take some snippets from your critiques and paste them anonymously into a document to distribute to the class, so that you too can benefit from each others' comments / experiences.
  What do you guys think? If there are any objections, please let me know and I'll make sure to exclude your comments if I send out this document.
